Betcha didn't hear about this rocket explosion.

In a pre-launch test of a VLS-1 booster at Alcantara, Brazil the rocket exploded on the launch pad killing 21 and injuring 20. The payloads SATEC and UNOSAT were destroyed. The explosion took place at about 1630 GMT on 22 August 2003. Reference: space.com
